4 years old male child was brought with complaints of headache and seizures. seizures were sudden in onset, generalised tonic clonic seizures and assoicated with incontinence. on examination, patient was febrile,103F ,and has neck rigidity. brudzinski sign was postive. a diagnosis of meningitis was made and physician was informed that his immunization status is uptodate. which isthe most likely agent to have caused the illness in this child?
a.streptococcus pneumoniae
b.haemophilus influenzae
c.group B stereptococcus
d.neisseria meningitidis
e.escherichia coli
f.listeria monocytogenus

If immunizations are UTD, then why Strep pneumo? To technically be UTD requires having received the pneumococcal vaccine. Should the answer be Neisseria?

since it is said that the child is fully immunised is is not h.influenza or streptococcus pnumonia
sreptococcus pnuemonia..vaccine is Prevnar and 4 doses are recommended.
1st-2mth,2nd-4mth,3rd-6mth and4th-by12-18mth.
and hib vaccine also 4 doses..2,4,6mthand12-15mth.
so i think the answer is neisseria meningitis
.

strep pneumo is the answer.
Aetiological agent of Meningitis varies according to the age.In neonatal period it is most commony caused by Group B streptococcus, and after that it is H.Influenza. This was the commonest cause of meningitis in the age group of this patient, before the discovery of H.flu vaccine.Now with widespread usage of H.flu vaccine, this has become less common and Streptococcus pneumoniae has become the most common cause of meningitis.
Neisseria meningitidis can occur in any age , but again it is most common in adolescents.
People older than 65 , it is again the Streptococcus pneumoniae which is the most common causative agent for meningitis.
Pneumococcus vaccine is recommened NOW A DAYS rouitnely from 2 months of age, but for some unknown reasons , immunization with this has not picked up .Moreover ,the protection offered by this vaccine is 70% at best.
I created this question basically to test the recall and to test whether or not student has learnt the fact that H.flu vaccine is widely used and that has changed the causative agent. This is a PREVENTIVE MEDICINE QUESTION ASKNG ABOUT INCIDENCE of a disease, a give away question.
