The patient is unable to distinguish the difference between a hot and cold test tube simultaneously applied to the ulnar side of the right hand and arm and the left leg. This deficit is in the same distribution as the pain deficit noted when testing sharp sensation. Pain and temperature sensation are tests for the a) Corticospinal tract b) Spinothalamic tract c) The reticular formation of the brainstem d) The hypothalamus e) Central vestibular connections

The correct answer is B Explanation The patient is unable to distinguish the difference between a hot and cold test tube simultaneously applied to the ulnar side of the right hand and arm and the left leg. This deficit is in the same distribution as the pain deficit noted when testing sharp sensation. Pain and temperature sensation are tests for spinothalamic tract function

While we are at it, corticospinal tract relay signals to alpha-motor neuron in the medullar spinalis and is involved in voluntary movement of the body. Reticular formation has 2 functions as far as I can recall. 1. Reticolospinal tract => stimulates inhibitory interneuron in the anterior horn of the medullar spinalis, inhibit alpha motor neurons. 2. Inhibition of pain. Pathway is like this. Raphe nucleus at mesencephalon => (serotonin - excite) reticular formation => (enkephalin - inhibit) sub. gelantinosa – i.e. inhibition of pain. Hypothalamus – control of virtually all endocrine function Vestibular – measure of equilibrium, reciprocal connection to cerebellum, vestibulospinal tract (controlling of extensors hence main function is reflexive balance.) - the only tract in the spinal cord that is ipsilateral all the way?

Btw I think there is a misunderstanding of terms. Spinothalamic tract conduct nociception not pain. Pain is a sensation. Nociception is signal, it’s always there if nociceptive free nerve endings are stimulated, however it can be inhibited thus causing less or no pain.
