Pt. has had nonhemorrhagic iscemic stroke visualized at 2 h by noncontrast CT of the head. What is your strategy?

Warfarin
Ca chan. blocker
t.p. factor
Infusion of Manitol
Hyperventilation

Sorry for typo mistake "ischemic" stroke.

tpa (tissue plasminogen activator )but it must be given within 3 hours from the beginning of the stroke

my guess is Ca channel blocker cerebral selective Nimodipine.
usually administered in SAH to prevent vasospasm.
In this Q pt has ischemic stroke - vasodilator will help (if not kill)

Now, the same Q but instead of nonhemor. will be Subarachnoid.
What is your strategy?

if not in fresh non hemorragic stroke, in wich case do you give tpa then?

What is the mechanism of hyponatremia frequently seen in SAH?

tPA within 3 h after oncet of symptoms has an advantage with respect to neurologic disabilities.

Sorry, could not see me007 Q while typed.
For Q 1 were nonhemor stroke ---- tp is correct (mms and paganini)
Q2--- Subarachn. stroke-----me 007 is correct

Can u explain Why?
Why not Manitol, me 007 will smile now

Ok, explanation.

1Warfarine--- not good, because it's not thrombolytic, it's anticoagulant
2.Ca cb----only for some type of strokes ( Subarachn) in which vasospasm may occur., but use of vasodilat. regvires a balancing of risk and benefits
3.tpA--- approved for ischemic stroke
4. Manitol---reduce IC pressure by osmoticaly removing water from the brain interstitium, used primary if brain edema.
