Case of Vomiting:
Always ask about LMP,
If young ask about DM type 1
Ask if projectile (clue to intracranial problem)
check for signs of GI and GU problems both

Don't forget to check for clubbing in pts with hemoptysis or breathing problems.

in case of menopause
ask about family hx of breast and uterus CA.
Ask about clotting problems in pt and family.
ask about sexual problems(offer counseling for partner)
Remember to tell the pt about the need for supplemental Ca and weight bearing excercises
Check for possible osteoprosis

in diabetic patient ask for clues to see if complications had occured(eye, kidney, heart....) also in social hx ask about excercising. remember to check sensation and distal pulses. remember to order lipid profile and BUn/Cr.

Check Carotid Bruit in diabetic pts and pts with history of syncope.

In pt with Vaginal bleeding don't forget to check for bleeding problems:
by checking other possible sites of bleeding like gums
examining the liver
and asking about hx. and family hx.

Remember Cocaine can cause bleeding, don't forget illicit drugs in cases of bleeding

In cases where you doubt abnl pregnancies like molar...:order both urine and serum BhCG


Don't shake hand with pt with shoulder or hand problems, don't try to change their position, remember to say you're sorry if you caused pain while examining.

in pt with obesity don't forget to ask about hx of steroids. and check for sleep apnea.

in pt with syncope remember to ask if any one witnessed the episode and what is their version of the story.

in case of sickle cell pt:
remember to check legs and fingers in physical. to tell them of importance of hypoxemia and hydration and ask if these were precipitating factors.

In a case of chronic cough always ask for intake of ACE inhibitors in a HTN patient.

ask for prior use of cocaine in a chest pain and vaginal bleeding cases.

Case of acute diarrhea: remember to ask about recent infections and history of antibiotic use.
in chronic diarrhea or acute diarrhea without clues to infection....ask about sexual practices and contraception.

In DDx of sore throat don't forget primary HIV(acute retroviral infection)!

In case of constipation remember Ca and DM tests in your workup!

In case of impotence remember to ask about claudication, pituitary tumors and spinal trauma.

In case of poluria: remember to ask about psychologic problems and if pt is taking lithium, also don't forget hypercalcemia in ddx.

always check for psoas sign and obturator sign in acse of abdominal pain.

always remember to look for cva tenderness in abdominal pain case.

make sure to ask about occupation and while doing the examination look for any mouth ulcers

in case of dizziness remember to ask relation to posture. (and order orthostatic blood pressure check in all limbs)

Do not forget:
Significant Alcohol consumption: CAGE queations
Unprotected sex with multi-partners: Counseling
Smoking: Offer help to quit
Abuse: Check for back-up plan, safety measures (e.g., availability of gun at home), offer social service support

What if the Pt has LLQ or epigastric pain? Still "always" go for psoas and obturator? Guys, your knowledge should be functional...
