Hi,
I have these questions:
1. regarding form L6. Is it necessary to fill in this form? I am an old graduate and this would a big problem for me. My postgraduate training was many years ago and some program directors are not alive anymore, others moved, ...
2. regarding Fee - is it $505 or $505 + 805? I want this letter only for application for residency in california.

Only you have to pay 505 $ now and when you will get the residency then you have to pay 805 $. Regarding L6 I don't know. Good luck

yes the L6 forms are required. It will be OK if you can get the DME (director of medical education) to sign them, it does not have to be the PD.
