You are seeing an 8-year old caucasian boy, who was diagnosed with cystic fibrosis for the first time. His other past med. hx. is significant for appy. at age 18 mo. He has a hx. of severe allergic reaction to eggs adn ibuprofen. His family hx. is significant for cystic fibrosis. Which of th efollowing statement is most accurate about thie patient?

A. He should be given infulenza vaccine only
B. He should be given pneumococcal vaccine
C. He should receive both influenza and pneumococcal vaccine
D. Do not give influenza but consider pneumococcal vaccine


Also:
What are ALL the Vaccines that are contraindicated in egg allergy? (is MMR one of them, and why?)

Does anyone know if its an allergy to the white or the yolk? or both?

D is right.

Patients with egg allergy are at risk of allergic reaction or anaphylaxis when given these vaccines (yellow fever, influenza, and MMR).

According to UW......they say that recent studies have shown that allergic reaction against MMR are more due to gelatin content of the vaccine rather than the egg, and thus many physicians recommend MMR vaccine even in the patients with egg allergy...

A test dose skin testing is recomended before administering the vaccine.

so.........for the purpost of the boards/USMLE.........give MMR or DON'T give MMR in a patient with EGG ALLERGY???

Hey colleagues,
can anyone see any difference between (B) and (D)? I think both of them are correct since the Pt has contraindications to the flu vaccine. This Q is from UW, and they say that CF pediatric Pts don't usually need pneumo vaccine since pneumococcus is not a frequent causative pathogen in this kind of Pts, but some physicians may consider it. I would give it, but again we have two options to choose from.
