D a picture of AF with Slight Signs of CHF ( Murmur cud be of Mitral Stenosis) THe other Option wud have been C Beta Blocker but their effects r a Beta blocker induced Low CO in pts with Arrhythmias. And also they can be used prophylactically later onwards. as they reduce Progression to Chronic Heart Failure Didnt Choose Amiadrone be cos of Toxicities Discontinue ACE wudnt be the right option in Such a Pt.

I'll choose D too because AF with HR ~94 /mins. is not too fast to start treatment of acute AF. This patient complaints about dyspnea but PE reveals within normal limit and no sign of CHF also consistent with Echocardiogram appearing AF with normal ST segments. I think it's chronic AF with stable clinicals,so giving Digoxin shouldn't be contraindicated.
