A 45-year-old woman has a history of asymptomatic ventricular couplets, for which she had been placed on quinidine. A week after this intervention, she experiences a syncopal episode and is brought to the emergency department. Her electrolytes are within normal limits. She is placed on a cardiac monitor and is noted to have a transient wide complex tachycardia with waves of alternating morphology. She appears to be in torsades de pointes. IT should say something about hemdinamic status of the patient.
