In the first 24h after MI which enzyme is best choice to detect infraction?

I thought Troponins were the best, but according to step up to the USMLE CKMB are the best.

it is in Goljan's lectures for step 1
CKMB rise 6 h- peak 24h - disappear 3 days
if it is present after it defines reinfarction

troponin 1 - rise 4h - peak about same - 24h, but lasts longer - 7 days, you can not diagnose reinfarction.

Thanks, but not taliking about a reinfract. I mean initially on presentation, if you suspect an MI would you do CKMB first of Tropinon

Buddy Troponin 1 is the First Enzyme to show up in the Presence of Infraction then Comes along CK MB SO if u r suspecting MI then Trop I is the best indicator n then Trop T n then CKMB Levels. Period. If u r still Unsure then look it up (online search) if something is different from above or the same then do let us Know.

You have an older version of USMLE.

Troponin came into real ER practice around 1999-2000 in US although Troponin has been around for a while.

By 1999, Troponin is widely used in the ER along with CPK-MB. But in 2007, no one used CPK at all in real life , real practice but older book still mention CPK
