hi all

I am in a fix..

i have matched at UT houston and they require a documen for name change........(my diploma and clerrships only have my first and middle name and no last name.......while my ecfmg cert has first middle and last name)

now they r saking me for the document for name change

how and from where do i get this document in the USA ?(

my passport has all 3 names
my GC and SSN have all 3 names
my driving license too has all 3 names

You probably need some document from your school showing that you are the same person. How did you get your medical diploma and transcript with just your first and middle names?
