A 35 y.o. female presents to your clinic with symmetric swelling and pain of her PIP,wrist and knee joints.Last week,her 5 y.o. son was seen for facial rash and fever. Her family history is significant for DM and gout in her father and RA in her mother.Her serum rheumatoid factor is negative.You recommend consecutive management with NSAIDs and one month later she calls you to report that her symptoms have decreased significantly. What s the most likely diagnosis?
A.-Borrelia infection
B.-Parvovirus infection
C.-Herpes simplex infection
D.-Rheumatoid Arthritis

Yup it shud be B cos if they hadnt mentioned the child history then it wud have been difficult to assume that its a Viral Inf. Yup they have Presented the Classical Symptoms.

Jean Robert it wud be nice if u post the explanation.

An adult who is not immune can be infected with parvovirus B19 and either have no symptoms or develop the typical rash of fifth disease, joint pain or swelling, or both. Usually, joints on both sides of the body are affected. The joints most frequently affected are the hands, wrists, and knees. The joint pain and swelling usually resolve in a week or two, but they may last several months. About 50% of adults, however, have been previously infected with parvovirus B19, have developed immunity to the virus, and cannot get fifth disease.
