A 30-year-old man is involved in a multiple car crash in which several people die. He is not physically injured himself, but in the days that immediately follow he feels numb and detached, dazed and disorientated, with physical symptoms of sweating and shakiness. The most likely diagnosis is:

Acute stress disorder is a short-lived but severe disorder caused by an overwhelming, psychologically traumatic experience. The symptoms develop rapidly, but tend to resolve within a matter of days. These include psychological symptoms, such as feeling numb and detached, dazed and disorientated, and physical symptoms, such as sweating, shakiness, palpitations and insomnia. Some patients will go on to develop post-traumatic stress disorder.

If there is persistent denial that the event has occurred, the patient should be cautiously prompted to recall the facts. A short course of a benzodiazepine tranquilliser and/or hypnotic may help severe agitation or insomnia.

B: Post-traumatic stress disorder

Isnt post traumatic stress disorder a possibilty .. as it too can last upto a MONTH .. and in this pt .. we dont konw how long its gonna last .. ? I mean .. whats the clue for A) Acute stress disorder

Addicudo post traumatic stress is diagnosed when the pt has these symptoms after a month of the incident. In the qn note : "A 30-year-old man is involved in a multiple car crash in which several people die. He is not physically injured himself, BUT IN THE DAYS THAT IMMEDIATELY FOLLOW he feels numb and detached, dazed and disorientated, with physical symptoms of sweating and shakiness."
