A 44-year-old woman has been admitted to hospital with atypical pneumonia. She has now developed a rash with blisters in the centre. There is no mucosal involvement. What is the diagnosis?

A Erythema nodosum

Erythema multiforme is an urticaria-like rash with blisters in the centre (Target cells or bull’s eye). It may follow a herpes simplex flare, Orf, Mycoplasma, bacterial infections, sulphonamides, penicillins, barbiturates and internal malignancy. Stevens��"Johnson syndrome is a severe bullous erythema multiforme with involvement of the mucous membranes of the mouth, eyes, genitalia, etc.

Orf is a viral disease that is widespread in sheep and goats. It can be transmitted to humans by contact with an infected animal or contaminated fomites. Reindeer have also caused similar lesions in humans. It is frequently seen in farming communities and meat handlers. Clinically, reddish weeping nodules are seen on the dorsa of the hands and the fingers that heal spontaneously in approximately 35 days
