Has anyone completed the forms for the il. temp. license, more importantly the CT form. Does anyone know how to fill this form or rather to whom do we have to send this form to have filled?
I would really appreciate any help that anyone can provide.

If you are from India then you send it to the medical council of India or the state medical council depending on who issued your license..

Do u mind sharing which state you got your license from in India? Have u already sent ur CT form to ur licensing board for certification and how long does it take for this process ?

I am registered with the medical council of India. Have not sent the Ct form out yet.. I think I will be sending it out sometime next week.. How aout you? where are you registered at?
