PDAs can actually close spontaneously or respond to indomethacin in premature infants. However, they rarely close spontaneously after infancy so we have to do a surgical closure. As for VSD, most of them close spontaneously by around age 2, especially if its a small a defect. However if it's a large defect, it must be repaired or else the patient is at risk for pulmonary hypertension and Eisenmengers (reversal of shunt), cyanosis, etc. Any other thoughts/ideas?

The other reason is that PDA is more common in premature neonates , and this defect causes a large amount of blood to be shunted as compared to VSD causing a lot of haemodynamic compramise.... premies cannot cope with this. The babies can develop high output faliure quickly hence they need to be operated.

yeah that is not wrong i guess ..but another answer if it was not absoulutly the correct is that in PDA the operation is just soo simple it is an extracardiac operation u know just need ..ligation of the ductus arteriousus...but in VSD it is intracardiac ...so if the lesion was moderate and the condition not deteriorating so we wait till the infant grows abit to make the operation abit easier ...but we do it early only if there is sever haert faliure .and in case of big defect

You all have good ideas guys. Another reason is the development of the Eisenmenger syndrome (pulmonary hypertension) which is faster in PDA than in VSD, if it is at all.

VSD is a left to right shunt as is PDA in VSD most defects will close spontaneously also in PDA you can give indomethacin for management which is not a surgiacl procedure
