A 65-year-old man develops oliguria and peripheral edema over a period of weeks. Urinalysis reveals hematuria and proteinuria; examination of the urinary sediment reveals red cell casts. Radiologic and ultrasound studies fail to demonstrate an obstructive lesion. Renal biopsy shows many glomerular crescents. This presentation is most suggestive of which of the following conditions?

(A) Anti-glomerular basement membrane disease

(B) Diabetic nephropathy

(C) Hypertensive nephropathy

(D) Lupus nephritis

(E) Minimal change disease

-systemic diseases like SLE, Wegenera granulematosis
a - anti-GBM - Goodpasture could be if he would be younger and had hemoptisis

Answer is A. Anti-glomerular basement membrane disease This is not the characteristic lesion of lupus nephritis. Nd this is a man!, lupus is more common in women

Lupus Nephritis is not a progressive disease its sudden oliguria and if it occurs without Hypertension then there wil be other signs such as acites or Pulmonary edema n pericardial effusion. there r 3 distinct points here where we can suggest RPGN of non Lupus type
1. History there is no history of Any Auto Immune disease or symptoms of SLE.
2. Progressive renal symptoms, in absence of Hypertension
3. Age factor

I had the same reason when i did the question cos the this man doesnt fit the criteria of Goodpasture's syndrome but he sure does for Idiopathic RPGN where type I is Anti-GBM without Lung involvement

AS we all remember its Idiopathic which has more of a tendency to occur in Elderly pts n mostly males.
