Kaplan Qbank USMLE

Need clarification.
In Kaplan the 1st line for Partial Simple seizures are Valproic acid and Phenytoin

For Partial Complex---- Carbamazepine.

Now, In Q bank I am reading that for both the first line is Carbamazepine.
Please clarify, asap, maybe my Kaplan Pharma is old

this wht i know

1. Simple Partial Epilepsy we used to give both Valporate n Phenytoin but Carbamazepine has become the Rx of choice in Both.

2. Carnbamazepine and Phenytoin have become the Rx of choice in most cases Although newer drugs such as Gabapentin and Lamotrigine are used now as adjuvants in the Rx

3. Valporic Acid is till used in Absence n Generalized Tonic- Clonic Seizures but is now avoided becos of hepatotoxicity Even then its still the DOC in both cases.

Therapy for generalized epilepsies
For patients who have multiple seizure types beyond partial and generalized tonic-clonic seizures, it is usually necessary to choose a broad-spectrum anticonvulsant with efficacy against multiple seizure types. Valproate remains a mainstay treatment for these patients, but some of the newer drugs, such as lamotrigine (18), topiramate (19), and zonisamide (20), are also efficacious against multiple seizure types and can be considered as alternative agents. Because of its tolerability, some physicians select lamotrigine rather than valproate as the initial drug of choice for some generalized epilepsies. For patients with pure absence seizures, ethosuximide or valproate is usually very effective (21). Lamotrigine can be considered adjuvant therapy or alternative monotherapy in patients resistant to or intolerant of the first-line drugs. Some early data suggest that levetiracetam might also be efficacious against absence seizures.

3Carbamazepine Hope they will ask new (page 150)

Sorry i dont have the New Kaplan I will Go For Carbamazepine as DOC in all the Partial types with Phenytoin.
