A couple presents to a clinic for workup of infertility after 5 years of unprotected intercourse. The wife denies any medical problems and notes regular menstrual cycles. The husband states that he has had chronic sinusitis and lower respiratory tract infections. Physical examination of the woman is unremarkable. Examination of the man is remarkable for dextrocardia. Further workup of the husband will most likely reveal

(A) azoospermia

(B) germinal cell aplasia

(C) immotile sperm

(D) isolated gonadotropin deficiency

(E) varicocele

Respiratory tract infections, Sinusitis and Infertility all indicate Immotile Cilia Syndrome.

My answer is C.
This patientn has Kartagener Syndrome.Symptoms result from defective cilia motility.

No dynein arm, sperm's tail is a flagella, so no motility. Defected dynein arms also affect cilia which are present in respiratory airways and this is the cause of sinusitis since the patient cannot clear secretions and therefore is predisposed to infections. Cilia is also necessary during embrionic development and that's the cause of dextrocardia, no rotation...
