I agree: it's B. Elevation of the affected hemiscrotum relieves the pain of epididymitis and exacerbates the pain of torsion (Prehn's sign). A normal cremasteric reflex makes testicular torsion less likely. Furthermore, on technetium Tc 99m scanning acute epididymitis shows an increased tracer uptake, while a defect in uptake in the scrotum occurs with torsion.
