I picked A when I did my NBME

not sure about B and E

E. Fever+ Flank pain >24hrs (if appendicitis --> peritonitis)

B--->appendicitis--->U/A normal( UPTO 5 WBC is considered as normal,>10 is abnormal)+ absent bowel sounds r/o pyleonephritis as a cause here..flank pain is common presentation for appendicits in pregnant ladies,coz the appendix can pushed up into the RUQ,as the uterus enlarges..

I go for ,pyelonephritis she has flank pain fever leukocytosis and increased urine white blood cells which most likely indicate wbc casts come on guys this was easy also this is an obgyn question not a surgery question uti in pregnant women is very common when a uti goes untreated it can lead to this pyelonephritis which presents like the question

16000 wbc is normal in pregnancy. 5 WBC in urine is also normal.

wbc 5 / hpf is too less for pyelonephritis

Aashi great job! Great explanation! I got this wrong on my test! Infact, this was the 1st choice that I ruled out bcos of its location!
