I have a stupid question. After I address the patient's name at the beginning, should I still call patient's name now and then during the meeting? I guess I will forget SP's name. confused

I had the same problem that i would be anxious to remember the pts name ...
so to be safe i greeted the pt with his/her last name politely when i eneterd and greeted the pt but clearly so that they make a note i called them correctly by thier name .
during the h/o or examn i said sir or mam so that i dont take the wrong name......

And do write the pt name on sheet ,before entering the room......after wards whenever ur summarising or leaving the room take a quick glance at the name on the sheet and you can address the pt again by name...

A good choice: to write the patient name BIG LETTERS at ther superior corner of the paper, try to do that while you practice so you will be used to find quickly and unconciously the name while your are doing some thing else
