pt with elevated WBC count and profound lymphositosis biopsy of lymph node shows hypocellularity in cortical and paracortical areas. Absence of which leukocyte surface mol coud be here and why a. addressins b. chemokines c. Ig family cell adhesion mol d. integrins e. selectins and what do you think is possible disease(s)

Basic defect is in interaction of lymphocytes wth high endothelial venules, i guess... So according to me.. the answer can be D- integrins.. n it may be a type of leukocyte adhesion deficiency... Not sure though.

Yup it has to Integrins and the Disease is Leukocyte Adhesion Deficiency

Alright i had to search for n found it right uinder my nose L-selectin is constitutively expressed on all neutrophils and monocytes, and on subsets of bloodborne T cells, B cells, and natural killer (NK) cells. It is well known to facilitate leukocyte rolling along the vessel wall, which is one of the earliest cellular responses to inflammatory stimuli or tissue damage. L-selectin is also required for efficient recirculation of normal lymphocytes into lymph nodes. The major selectin ligand, sialylated Lewis X (SleX), is missing in Leukocyte Adhesion Deficiency (LAD Type II). SleX, or CD15s, is a blood group tetrasaccharide. Both the sialic acid and the fucose moieties of SleX are needed for binding to the selectins; a defect in fucose metabolism is present in LAD II. LAD Type 1 has Beta integrin deficiency the child does not survive for longer than 1 yrs

sorry, didn't see all that replies. It is selectins, new_n_lost explanation is about the perfect one. the stem qs described 4 y.o. boy, i think it is some not very common disease, there is no ans for the disease. there are lots of lymphocytes in the blood, but not in limph nodes, means they can't get into l/n Selectins - receptors on lymphocyte surface that mediates their binding to the endothelial venules of l/n. It is how lymphocytes enter lymph.node. (also initiate adhesion in case of inflammation) addressins - complementary to selectins receptors on endothelial cells, can cause same symptoms, but qs about lymphocyte receptors. integrins mediate cell-to cell interaction, extravasation and binding to intracellular matrix
