don't we use isotretinoin for nodular acne?

  #4

Kaplan says use it if severe nodulocystic acne having scarring

UW says both nodulocystic or scarring

for this q, A or C are possible, but I prefer C because this is first time treatment.

Definitely C. Oral isoretinoin is only used for nodulocystic acne, the most severe form of acne. This person has mild inflammatory acne. If topical retinoids or topical antibiotics were also choices, it would be tough because any of these could be used, but I doubt they'd give both as answer choices, and from this question it is clear that they consider topical benzoyl peroxide as the first line. If the patient just had comedones, the answer would be topical retinoids, because they are comedolytic and I don't think that topical antibiotics or benzoyl peroxide would be appropriate.

i wud go with C ..... 1-2 mm white papule ..... are white comedones which are now infected to become red papule .... also on forehead there are nodules ( few) so initial therapy will b topical benzoyl peroxide .....

if nodules were not given in case and white comedones are there then u go for topical tretnoids .... they act as exfoliant ....

oral retinoids are for nodulocystic acne ....

topical metro for rosacea..

I wrote it A in my test thinking she has "nodules". but now I doubt my answer. Can anyone discribe how acne is staged? Like which one to call mild, moderate or severe with any specific criteria. I would really appreciate it.
