A 60-year-old man presents in your office with failure to thrive. He has lost 2.27 kg (5 lb), has a poor appetite, lethargy, hair loss, constipation, and sensitivity to the cold. His blood pressure is 100/80 mm Hg lying down and 80/60 mm Hg standing. You find it difficult to palpate his thyroid gland. His stool is negative for occult blood. His deep tendon reflexes have a delayed relaxation phase.
