A 45-year-old woman has a history of asymptomatic ventricular couplets, for which she had been placed on quinidine. A week after this intervention, she experiences a syncopal episode and is brought to the emergency department. Her electrolytes are within normal limits. She is placed on a cardiac monitor and is noted to have a transient wide complex tachycardia with waves of alternating morphology. She appears to be in torsades de pointes. B. Stop quinidine and observe. The pt is not compromised and in Torsades, the precipitating factor should be treated first.
