b. never give up. might be this attempt will have success. smiling face

smoking effects on GI.
1.GERD
2. delayes the rate of ulcer healing
3. risk factor for stomach adenocarcinoma

also smoking is MC single preventable cause of cancer and MCC of premature death in the US.

excuse me but prob he is ahving is most likely that of DU so lifestyle shoud be recommended keeping his present condition in mind and may be coffee quittin or milk intake????????

bcoz according to me there's no use of advising something that the patient refuses to do.
If it ws actual life, icould try to counsel him fr de-addiction.. but given the situation in the Q... A.

I'd also stress on quit smoking and even more i'd present the complications and the risks the pt is taking ! I'd talk about support groups & counseling on this matter that would strongly help quitting the habit ! even more i'd offer my support on this matter!

i thought you were sitting for the test today...
anyways, i think it should be coffee. smoking or not, but coffee should be excluded in gastritis.

I wanted to sit for the exam, but apparently to reschedule when the test has already been started takes getting a new permit, and investigation by the nbme. So i am waiting for my new permit.

When I saw this question my first inclination was to choose smoking, but like indidoc said there is not point.

this most likely means that the answer is D. Salt intake is shown to increase the chance of gerd by 70 % . This info is all over the net, but not in my books( books are old)

look at this q from patient-doctor relations side.
no one quitted smoking with first attempt and pt always will tell that he never.., and still as a doctors we have to try.

you right now one does on the first try , but I had a question from a q bank before with an intervention that a patient would not do, and the answer said if the patient absolutely refuses to do something then we must find the intervention that will work.

agree on this one, ia had couple of questions during my preparation and the rule was: if Pt doesn't want to do something look for other choices, but help the Pt. you can not make Pt do something that he/she doesn't want to

i can add that in CS exam you have to approach this pt in this manner.. keep trying to expose the risks he is at, the complic...offer help .. counseling, support gr, ultimatelly your own support... thas why i asked the source of this Qs, i guess Ancylostoma dont wanna tell us more grin unless pm

Buddy i just found a link in which Coffee is implicated but as far as Salt intake is concerned there is no definitive implication SO i wud say its a toss bw coffee n Salt intake. The Apparent reason for leaving coffee is that the pts try to avoid things which induce more acid in their Stomach.

you right its a little bit of a tossup, the study on salt intak also noticed that heavy coffee drinkers had a decrease in gerd. whatever that means it tilts me too salt in take.

Goljan says that 2-3 years after a study its fair to see on the exam.

True abt that Half of the researchers say that Salt intake shud be decreased while other half say coffee shud be decreased. Now they still havent made a conclusion yet so i think i will stick to Coffee as it is proved

keep thinking that bcs he is beginning to experience new symptoms, doc just have to tell him that it is not going to be better if he will not quit smoking.

same here... i'd first remind about the bennefit of quitting smoking !!!!!!!!!!! It is a CS case guys smiling face
