A 44 yo M complains of vague RUQ discomfort that he has had 1 mo. He has no other symptoms, and except for enucleation of 1 eye at age 21 "for a tumor", he has been in excellent health all his life. He exercises regularly and neither smokes or drinks. The only findings on PE include the artificial eye and a tender, enlarged and nodular liver. Viral hepatitis serology is negative. It's A... melanoma. Only a melanoma can come back as a metastatic cancer after a gap of so many years.

The enucleation of 1 eye 23 years ago was du to a melanoma
Melanoma has this unique caracteristic of metastasis after a long time and also has this strange place to give metastasis ( RUQ disconfort = may be in the gallblader in this particular patient )
