A renal biopsy is performed on a 33-year-old female
who has had increasing renal failure for the past week.

But the prob is.. isn't ATN reversible mostly??

ok.. found an explanation fr this one.. kinda...

ATN occurs usually by ischemic change which quickly leads to irreversible cell injury (Which is asked here) leading to coagulative necrosis n sloughing off of the tubules...
The reversible nature is bcoz some cells stick in there n regenerate later on..

Ok Here r my 2 cents
Plasma Membrane Blebs n Mitochondrial Swelling r found in Less Severely Ischemic cells in ATN who r going to or abt to go undergo Apoptosis.
While Nuclear Fragmentation is the Last stage n confirms the diagnosis n Therefore representing Both Cell Death n Apoptosis in ATN.

necrosis---sign--->karyorrehexis(frag of nuclear) isnt it sufficent??

Necrosis = irreversible cell injury

and only cell fregmantaion (karyorrhexis) is a sign of irreversible injury among the choices. All others are reversible.
