She was planning to get a job, but has been staying at her parents' house, often sleeping muchof the day and wandering at night. Her speech is a bit odd, with tangential answers to some questions. She has no relevant medical history. She has not smoked marijuana for 2 yrs and never used other drugs. She drinks only rarely on big social occasions 2) 49 yr female complains to her primary care physician that she is under intolerable stress at work. Her manager seems to be trying to make her fail, and her coworkers treat her badly. She wonders whether they may be acting under the instructions of FBI, who have reams of files about her since she worked for Social Security Administration 10 yrs ago, She has few friends, but is close toher family and active in hiking club. She feels discouraged and unhappy, though she enjoys her time away from work; vegetative symptoms are limited to disrupted sleep on Mondays before work and some fatigue. She is overtalkative and circumstantial in trying to prove she is the target of investigation, but otherwise logical and coherent. She denies drug or alcohol use on the grounds that 'You never know what someone might put into your drink' 3)50 yr man is brought in by his wife because 'He's going off again'. She reports that he is not sleeping much, talk incessantly about plans to increase his business, and has run up unusual credit card debt from impulsive purchases. He is a social drinker, but lately he has been opening beers and leaving them around the house after only a few sips. He seems to be hearing voices, though he refuses to answer questions about this. His behavior includes starting tasks and not finishing them, eating furtively at odd times, rummaging in storage areas, and making lists. His wife reports his presentation is similar to an episode 5 yrs ago. She thinks he has been hearing voices for months, but his sleep changed only 1 week ago and he was acting fairly normally until 3 weeks ago 4) 30 yr man who works as custodian, though has a college degree, presents to your office. He lives alone and has never had an intimate relationship. His chief complaint is that 'I was told to come here'. He says he hears voices that tell him what is safe or not and what he should do. He admits to funny shock'like sensations in his body and says that 'they' have planted electrical wires that cause him to have an orgasm or to experience pain anytime 'they' want to. He smokes a joint of marijuana perphaps every 2 months and drinks fee beers roughly as often. He presents himself in a stilted, robotic way, as though he were responding to military commands. He says he has had the schoks and heard the voices 'for a long time' and was hospitalized when he told his family about them 2 years ago 5) 17 yr high school student wo has been in and out of psychiatric treatment for 5 yrs presents to your office. He was an isolated, quiet child. In adolescence, he gradually developed a thought disorder, keeping a detailed diary that is almost incoherent, filled with references to superheros and sports stars. His answers to questions are tangential, at times frankly illogical. He admits to auditory hallucinations. He smiles inappropriately and seems detached from surroundings. He seems to lack awarness that anything is wrong. He also lacks initiative, though he will generally accede to requests that he bathe, or put something away , or take his medicine
