Yes you should ask the Sp " i need to examine your chest and heart . Is it all right with you sir/mam. Can i untie the gown for examination." Drape the pt first then untie the gown ,Help the pt to take the sleeves out and lower the gown . Mostly pt will not move when you ask them to untie ,you have to untie and lower it gently asking if they can take the hand out of the sleeve also.

You don't have to bring down the SP's gown more than the mid arm level. If you do it you will certainly realise how awkward it looks especially when performing the female heart exam. What you can do is to bring down the gown to the mid arm level and shift the gown slightly forwards then insert the stethoscope and listen to the 4 sites. In most of the female cases you will definitely have to only auscultate the heart and lungs quickly. Remember the PE is not the most important part so don't waste time on it. Hope I gave a response to your question.

If a man there is no prob... if the patient is a lady, then you should not uncover the whole chest... it will cost you points. Try to uncover the back only by letting the gown tied at the neck, arms out of the sleeves and let the gown hanging in front of the patient.
