Q: Conduct disorder vs Oppositional

A single mother brings her 10-year-old son to the office. She says that she is "absolutely fed up" w/ his behavior and is unable to control him. The boy frequently gets into fight w/ his siblings. neighbors and classmates. When asked to help w/ household chores, he refuses. He is very short tempered and argues frequently w/ his parents and teachers. A few days ago he got into an argument w/ an neighbor and in a fit of anger he "grabbed a marker and wrote an obscinity on her front door" .
What is the most likely diagnosis?
A Conduct disorder
B ADHD
C Tourrett's disorder
D Oppositional defiant disorder
E Antisocial personatlity disorder


Please explain.

both have similar symptoms but Oppositional defiant disorder: disobedience, no fighting

Oppositional defiant disorder --meaning the kid has problems with authority figures so I go with answer -d...

Conduct disorder Ithink more of a young criminal indulging in various criminal activity

you cant always agree. its better to state a crisp opinion... Although he is hostile, vindictive and argumentative, he did not break rules!!!

I think it's A because of the fighting.
Worth remembering: "conduct disorder" for patients under 18, "antisocial personality" after 18 years old. Same symptoms.

I go w/ A since based what I learned in Step 1 patients w/ ODD usually only have problem w/ authorities but not w/ peers; but the patient in this case seems to have problem w/ every1.

However, the answer provided is D.

guys, say what you want this how i see it"

conduct disorder--- young criminal, fighting people he DOESNT know. 2)destroying not vandalizing property, 3)totally dishonest 4)rule breaker eg. fire setter, school truancy, rape or robbery. genetic related XYY?.

ODD-negativistic, arguments, temper tantrums, fights people he knows, does not break the rules does what he wants to do doesnt like authority. eg. one parent households?

I absolutely agree with mjl1717--->the q had more than one clue pointing out that its opp.defiant disorder--->multiple times the q points that the child has PROBLEMS WITH AUTHORATIVE figures like principal , teachers and parents, ie) hez upset with guys who tries to control him,that is those guys who mingle with him and NOT STRANGERS..they arent as cruel as conduct disorder guys..in a typical hx of conduct disorders they will mention fire setting behaviour, cruel to animals, 100% law breakers,these guys are considered as juvenile criminals..opp.defiant are bad tempered guys,but they arent cruel or doesnt have homicidal behaviour...

I don't think so, Oppositional defiant disorder patients are OK with sibling and classmates (their fellows).

It's D

To diagnose conduct disorder you need 3 out of the following criteria:

1. Aggression to people and animals
2. Destruction of property
3. Deceitfulness or theft
4. Serious violations of rules

For me it's D both because obviously Conduct disorder's the dignostic ccriteria posted by fongch are not met in this case and also because he didn't beat or try to kill the neighbour but simply wrote an obscenity on his door which is bad but not that bad... I think that if the right ans had been A the writer should have given some more infoes like fighting and beating the older neighbour.

Anyway it's true that it's a tricky q

yes its oppositional defiant disorder....remember its a milder version of conduct disorder and interestingly some with this disorder may eventually develop conduct disorder.
anyway answer is undoubtedly D

--just had this Q in UW and the answer is ODD...
However the explanation says that ODD is a milder version of Conduct disorder and they do not violate the rights of others... But in the question it mentioned that he valdalized and got into fights ---isnt' that an example of violating someone else's rights?
---isn't vandalism an example of destruction of property also?

Maybe by "destruction of property" they mean setting fire or breaking into pieces... And although this patient fights, he's not cruel...

Yes, it was a good question, and although he does seem like he could be conduct disorder, it is more a presentation of Oppositional Defiant Disorder (a milder form of Conduct disorder).

I guess the key is that Conduct disorder is like a mini-criminal who breaks the law and has no regret-He is on the way to Antisocial Personality Disorder when he turns 18. Whereas Oppositional Defiant Disorder is more of temper, argumentativeness, no regard for authority figures, etc...

yes i hit upon this question yesterday in uw..its ODD..uw gave a good d/f between conduct disorder and odd..yes for conduct disorder u have to break laws fire setting ,violating rights of another etc which odd does not..
