Dear Dairy,

I will be writing to you daily to reflect upon my preparation. You will keep me away from hobbies and interests. You will help me manage & concentrate on the IMPORTANT thing of my life! - the ONLY ONE of them being USMLE!

I have studied hard for a few months and then something comes up and i am washed away from studies for months on stretch and by the time i realise and muster the courage to start up again, i am back to square one. EVERYTHING NEW! This may sound ridiculous to some people , but its true! And unfortunately it has happened to me not once but many times. I am supposed to concentrate but easily get distracted , either because of responsibilities or hobbies. I know the latter is totally my fault and I am to be blamed for it.

I have never attempted the test but scheduled my test twice with a goal but never even completed once. I have a full-time job and an obviously old grad.

Dear Diary - keep me focussed and concentrate on my life's sole aim and goal. Give me the power to complete little goals every hour and every day. Tiny steps make a big goal achievable.

My goal for today is to finish off 28 pages of Goljan lecture notes + Pocket Robbins (where needed) and Goljan audio for that content. See you tomorrow! nod

shaking head I did read a total of 10 hours today and woke up till 2 am nod . I knew that reading 28 pages of Goljan in 10 hrs is not a joke for me but wanted to try. Lost 2 1/2 hrs in grocery shopping. I read 20 pages with references into physiology. Overall satisfying! GL tomorrow. Good ight.

Tomorrow's goal is to read 30 pages of Goljan and do Robbins review Female rep system. GL too! nod

I read little more than 20 pages in less than 10 hrs. Could not sleep more than 10 mins afternoon. disapproval Should have slept for an hr! ok. Tomorrow I will be entering the unchartered territory of Muskuloskeletal system. 20 pages tomorrow would be one too many for me. nod

Motivation is the key!

Why are ACTH levels increased in ectopic Cushing's inspite of increase in Cortisol?

Dear journal - yesterday I read for 9 hrs. I could not start Musculoskeletal system but I was able to review Endocrinology from Goljan. I am not able to read more than 20 pages from Goljan (with audio). I don't know if I am not getting it or it is ok. I know I used to read even 40 pages during phys, Biochem...

One more concern has been my son. He sleeps next to me and he has been moving a bit too much in bed waking up this already anxious usmle soul. For the last 3 days, I did not sleep more than 6 hrs. I need to address this issue as I believe that hard readers should sleep at least 7 hrs for consolidation of read material.

Today I will finish Musculoskeletal system from Goljan notes with audio.

Dear Diary,

I read well 3 days before yesterday. I went into lazy mode yesterday. I planned to do quite a bit of reading. The local TV interview prep. upset my rhythm. Is working at work not part of what I expect? With the work I do, I expect such obstacles for sure. Hence, it is important to just brush aside such hindrances and move on with the prep. Studying continuosly for 3 days doesn't give me the lease to waste one day.

Let's see what I can do today. GL!

I know exactly how you feel! Dont give up we are all in the same boat! Together we need to find strength in conquering this exam...It can be done. Good luck and God speed!

Thank you Gimbugie.

I as usual did not read anything yesterday. Why do you think it happens? It is a disease. I'm not shy to tell it is. I read well for 3 days and then skipped 2 days. On and off. Eventually, something else comes up and I give up for 2 or 3 months. Then, start again. Huh?

Ok. Anyway, since we are here not only to finish off step 1 USMLE but also find a cure to this. I'm sure I am going to read well for the next 5 days (with the weekends coming). So, let's start preparing myself for the next 10 day stretch. Jobs, kids, responsibilities are all excuses. Residents work for 80 hrs/ wk or more - how? Because they have to. Here it is the same that I may have to do except I am shirking reading.

Enough. Now, today it is going to be Musculoskeletal system from Goljan Path + audio. Cheers!

i dont know if that was a question that u wanted an answer to or it was just a point to ponder for the rest of us but the answer is nevertheless,right here.any ectopic secretion of hormone is independent of negative feedback.that goes for ectopic PTH secretion also in case of small cell carcinoma of the lung.even ADH levels dont go down no matter how dilute your plasma is,when ADH is secreted ectopically.the same goes for tumors.these paraneoplastic syndromes are secretory tumors in their own right and so cannot be suppressed by normal phenomena.
