I need an information here: Where should I get my statement of need from? Italy where I'm permanent resident or homeland? I read in the ECFMG website that I must get it from the country of last residence. What if I want to go back to homeland for the 2 years post residency?

You need it to get the J1 visa from ECFMG. More info @ ecfmg.org
