A 28-year-old woman, who is 29 weeks' pregnant, is evaluated because of progressive dyspnea of 3 weeks' duration. Cardiovascular examination shows elevated jugular venous pressure and a heart rate of 100/min. The apical impulse is prominent, and a parasternal impulse is present. An opening snap and a grade 2/6 diastolic rumble with presystolic accentuation are noted in the fourth left interspace. No systolic murmur is noted. S2 is accentuated. An electrocardiogram shows sinus tachycardia, left atrial enlargement, and right axis deviation.

I d still say TR...based on murmur. Though some sigs of CM present. Diffuse AI, tachy, cracles.

Besides the mechanical characteristics of a mumur, its important to consider accompanying signs and symptoms. You yourself have listed signs quite indicative of CM, is thr any particular reason why u would still say TR?
