A 70-year-old female resident of a nursing home is evaluated in the emergency department because of decreasing mental status and hypothermia. She has a history of stroke and is currently taking only aspirin. She has been able to eat on her own, and there have been no witnessed aspirations. She has not been treated recently with antibiotics.





Her leukocyte count is 12,000/μL, and her hemoglobin is 12 g/dL. Serum electrolytes are within normal limits. Chest radiograph shows a small interstitial infiltrate in the right lower lung field.





Antibiotic therapy for which of the following organisms should be included in her treatment?


( A ) Anaerobic bacteria
( B ) Aspergillus fumigatus
( C ) Enteric gram-negative organisms
( D ) Pseudomonas aeruginosa

But still I am not comfortable with MS changes and Hypothermia, how long? Gram -ve can be correct if a stroke has happened.

i think i ll go fr anaerobes as i m thinking of aspiration coz of 1. decreasing mental status 2. rt lower lobe of the lung 3. stroke.
GNB are more prevelant in intestines.as far as the stomach is concerned...anaerobes r more...

she is residing in a nursing home...it doesnt say tht she s bn intubated or catheterised or any other in dwelling cath...so y pseudomonas? plus this is probably not the nursing home we know it as...this seems to b the old age home more prevelant in US...thts wht i make out of it...pls correct me if i m wrong...
