A 56-year-old man comes to the office for evaluation of his diabetes. He is well maintained on a sulfonylurea and metformin. He is a nonsmoker and has no history of hypertension or coronary disease. His father is the Jedi master Yoda who lived to be 900 years old without coronary disease. His glucose level today in the office is 135 mg/dL (normal 60-110 mg/dL), hemoglobin A1C 7.2% (normal 4-7%), and his LDL is 145 mg/dL. What is the most appropriate management of this patient?

(A) No further therapy indicated
(B) Step 2 diet, restricting lipid intake alone
(C) Niacin
(D) Statins

You give statins. He's got diabetes which is as bad a risk factor as coronary disease itself (the so-called cardiac equivalent risk factor). The recommendation for people who have cardiac equivalent risk factors (which are diabetes, carotid artery disease, AAA, and peripheral arterial disease) is that their LDL levels be less than 100. You can keep them off drugs (and stick to diet and exercise) upto an LDL of 130. He's got an LDL of 135, so we go for statins.

yes must give statins...
Diabetics must recieve lipid lowering drugs if their LDL levels are above 100...

One reason statins are given is because even with possible side affects// they DECREASE mortality!!!!
