An 8 yo boy who recently emigrated from Haiti is brought to the physician 's office because he has had reccuring episodes of high fever every third day for the past 2 weeks . He is complainning of headache , generalized achiness and abdominal pain . Temp is 48o C HR 140 X " BP : 90 / 50 mm Hg . He is ill appearing and jaindiced . He has no nuchal rigidity . Abdominal distention is significant for a large , tender , spleen .

Lab studies shows now :

Hb : 7. 5 g / dl

Htc : 22 %

leucocyte count : 3500 mm3

segmented neutrofils : 35 %

lymphocytes : 20 %

monocytes : 45 %

platelet count : 350 000

total billirubin : 10 mg / dl

direct billirubin : 1.5 mg / dl

Which of the following Lab studies will most likely reveal the diagnosis ?

also weakness, splenomegaly, and ironically possibly herpes labialis! any one want to comment on the prophylactic Rx!
