I have a question about the L5 form in the california letter (ptal). What if you don't remember the exact dates of attendence.

You should contact your school for that piece of info. You have to keep in touch with them for your L2-L5 forms as well as your MSPE anyway smiling face

Hi can u please tell me wether the clinical instruction dates asked for in the form L5 includes only the rotations done as an intern or also the clinical postings done as a student too.please help!!!
