Aspergilosis is a hypersensitivity pneumonitis with a normal CXR or miliary nodular infiltrate.
you could also have alveolar thickening and granulomas.

Tb may be the right answer but it was not the most convincing scenario

No hemoptysis, night sweats, PPD, nothing-just a guy going to a 3rd world country!! also I thought Tb went to the apices?? also the q is not that legible

Decreased breath sounds in the LUNG BASE--->is due to PLEURAL EFFUSION in this case..
All other choices can be easily ruled just based on the XRAY findings..hilar L.N and pleural effusions are more suggestive of T.B than any of other choices in the q..

Albania is considered as one of the hubs for T.B, malaria and AIDS
This is a case of T.B until proven otherwise..
