how can we distinguish pre renal cause of acute renal failure from acute tubular necrosis?

Ischemic acute tubular necrosis has the cause than pre renal in the fact they are both secondary to decrease renal perfusion. Signs of orthostatic hypotension and hypovolemia may suggest.
In toxic acute tubular necrosis find and remove the offending agent and the kidney will go through phases of recovery with supportive measures.

urine osm in prerenal >500 in ATN < 350

urine Na+ in prerenal <20 in ATN >40

FeNa in prerenal <1 in ATN > 1

BUN/Creatinne ratio >20:1 in prerenal and 10:1 in ATN
