Mr. and Mrs. Smith, a famous couple in the area where you practice, brought their beautiful blond and blue-eyed baby to your clinic. The parents are concerned that the kid is not growing up as fast as other kids in the locality !! Mrs. Smith also complains that the baby sometimes smells so bad that it`s hard to cover that mousy odor with any brand of kiddy-colognes. Mr. Smith is worried if the kid had any problem during delivery, since the kid was born outside US where, he doubts, if routine pediatric screening tests were performed. Which of the following statements is true about the disease of this kid ? A.The kid would have a high level of phenylalanine in urine B.The enzyme deficient in the kid carboxylases phenylalanine C.The possible reason for the developmental delay is high phenylketones in blood D.The mousy body odor is caused by phenylacetic acid E.A negative Guthrie test will confirm the findings And bonus q. what`s the name of the kid ???

D I thought it would be elvated phenylalanine in the blood and phynlketones in the urine. I go with D. I have read that phynelyacetic acid(in usmle world q I think) causes the mousy odor. It would make a wonderful cologne. I dont know who Mr and Mrs Smith is if they are actually people and not actors. If you talking about the movie, Ms smith- Angelina, is totally hot. Got to love the women. The kid was shiloh and he was born in Africa I think. Who is joe smith the third?

A) Incorrect, phenylketones are increased in urine (phenylacetate, phenylpyruvate and phenyllactate), not phenylalanine B) Incorrect, the enz is Phe hydroxylase, not carboxylase C) Developmental delays are caused by high phenylalanine in blood, not for the ketones E) A positive Guthrie test will confirm the diagnosis, not a negative one D) correct. The name of the kid doesn`t really matter .... it was a bad joke ... anyways, Anky is right, and she was born in Namibia. I`m voting for constitution ave. Anky.
