A 14-year-old female presents to her family practice physician for a school physical. Her only complaint is pain in her left calf and ankle. She has no significant medical history. Family history is significant for a father who died at age 30 of a myocardial infarction and a paternal uncle who died at age 36 of a myocardial infarction. Her paternal grandmother died in her 40 s of an unknown cause. The physical exam reveals soft, yellowish elevated plaques on both of her eyelids. The patient also has a tender nodular elevation about 1.5 cm in diameter in the area of the left Achilles tendon. Her lab results are significant for an elevated low-density lipoprotein (LDL) of 821. Which of the following is the most likely etiology of this patient's elevated LDL? A. A defect in bile acid synthesis B. A defect in HMG-CoA reductase enzyme production C. A defect in the LDL receptor D. A defect in the storage of cholesterol esters E. A diet high in fat
