What is the next step in management of a palpable breast mass in a woman over 40 yo

FNA or mamography ?

Just wanna know what u guys think because Kaplan say mammography and Goljan 2 and some others surgery books say FNA ..................

Kaplan and UW says Mammography AS THE BEST INTIAL SCREENING TEST, for NON-PALPABLE, masses at the age of 40 and age of 35 for pts with family hx, BUT once a mass is felt (PALPABLE) during the examination, u shld get a tissue bx (FNA-C/EXCISIONAL/INCISIONAL), coz MAMMO is just an ADJUVANT for p/ex, is good for finding out occult lesions which may escape during P/EX and ofcourse micro-calcifications..once the lesions are palpable ,mammo doesnt offer much benefit in the dx.

In this Particular case( 40 yr old female with PALPABLE BREAST MASS) u can go for FNA-C, and if it was postmenopausal female WITH A PALPABLE MASS--> invasive procedures are better as intial step for obtaining tissue, coz the incidence of malignancy for that particular AGE group is HIGH..

In Kaplan lecture , the prof said that you still have to do a mammography in palpable mass in woman over 40 to see what you ' re dealing with and examine at the same time other tissues

Goljan Step 2 gave exactly the same explanation as Aashi gave me ( Thanks Aashi ) , which by the way I agree with

Any other sugestions , PLEASE ?????????????????????
