Hi..I just want to make some concepts clear...Following questions may look very simple..However,inputs are appreciated. 1.Why does in pineal gland tumor cause precocious puberty? 2.In pinealoma,it compresses only the pretectal area...Is accomodation affected in Pinealoma?(pupillary constriction in accomodation reflex is affected) 3.iF SHIGELLA CAUSES hus,wHICH IS THE mcc - sHIGELLA OR e.COLI? 4.How to differentiate betwen infections of Plasmodium vivax and ovale? 5.In lambert-eaton syndrome ,Why is there improvement after repeated contractions?? 6.Does splenomegaly present in ITP??Why does extra vascular hemolysis causes splenomegaly?? Thanx

1)? 2 I remember clearly that vertical gaze is affected, thats how I remember this tumor. I havent seen a questions without it. 3 shigella and ecoli both cause it, but I dont think you should concentrate on MCC, maybe presentation, eating burger etc = E coli. If I was going to choose one in the US it would be Ecoli 4 Look at kaplan micro book, they give you a column on what each organism looks like 5 Tell you truth dont know, I know its an ab to calcium channel, get better, paraneoplastic. If the ask why, i'm screwed 6 Splenomegaly in ITP- work Hypertrophy - USMLE world question

Thanx anky ..Is there any difference in the clinical presentation between Plasmodium vivax and ovale??

1. didn't know it before 2. same as Ancylostoma, but i think it is not affected, because for that you would have to get involved oculomotor, which is not 3. E.coli without any doubts. 4. still to come for me (don;t know yet) 5. in ITP there is no splenomegaly, petechia is not hemolysis, it's due to low platelets but in extravascular hemolysis - spleen is overworked and overloaded with degradation products of destructed cells

Looks like I was crapping, There is not typically spleenomegaly in ITP, if there is it exludes the diagnosis of ITP and you should look of for other diseases. Spleenomegaly is found in extravascular hemolyis because of work hypertrophy. in a child it doesnt exclude the diagnosis as it may be there in 3-10% depending on the article read.

1. Parinaud syndrome, a condition secondary to compression of the tectum, is the most important clinical presentation of pineal germinomas. The triad of Parinaud syndrome includes palsy of the upward gaze, dissociation of light and accommodation, and failure of convergence. 2. Pineal region tumors in children can present with endocrine malfunction. Hydrocephalus or concurrent suprasellar tumors can cause diabetes insipidus. More specific endocrine syndromes can arise from secretion of hormones by germ cell tumors. Pseudoprecocious puberty caused by beta human chorionic gonadotropin (bhCG) can be observed with germ cell tumors in either the pineal or suprasellar region. 3. In North America and Western Europe, 70% of cases are secondary to Escherichia coli, In Asia and Africa, it is often associated with Stx-producing Shigella dysenteriae 4.There is no clinical precedent for distinguishing the 2 forms of malaria except histologically i guess atleast thats wht i have read.
