Why is the TIBC in anemia of chronic disease decreased? The serum Fe levels are decreased, so TIBC should be increased, not decreased... ?

I had the same question before. But Goljan review says, if my memory is correct, the serum Fe is low, but total Fe not low, they are mostly trapped in the bone marrow.

i go with webjee. In chronic disease, serum iron gets taken up by cells or bone marrow (?), and so total Iron is normal (or high), and TIBC decreases. Both Goljan and dr.Francis (Pass program guy) say the same thing.
