47/M accusing his wife for having an affair with the neighbour.It progressively escalated to the point that he was accusing her for cheating on him with their priest.HE STARTED GOING WITH HER EVERYWHERE and locked her up inside the room when he goes outside.His mental examination is not significant.Diagnosis?

A.delusional disorder
B.Paranoid personality disorder
C.paranoid Schizophrenia
D.delirium

A. Delusions with no hallucinations , Persistent persecutory delusion > 1 or 6 months, I forgot how long its required to be there before you diagnose it.

B paranoid personality disorder? I chose this as the q says that is mental exam is not significant for any abnormality.

And it`s not B b/c it`s fixed and focal and mental state exam not significant ?

The answer is A...Delusional disorder...

Clozapine,fiXed and focal refers to the husband being suspicious on her wife ..The activity of the husband is on only one aspect and so is it fixed?

To diagnose a paranoid personality disorder,should the husband have mistrust on several aspects?Please clarify me.

Paranoid personality disorder also may be confused with delusional disorder.

Two significant characteristics distinguish these disorders. In paranoid personality disorder, the hostility and paranoid thinking most often are generalized and affect multiple areas of the person's life. For example, the patient may be jealous of the spouse but also will exhibit hypersensitive thinking at work and in other areas. By contrast, the psychotic thoughts of delusional disorder usually are focused in a single area with remarkable preservation of other areas of thinking and functioning.

Another feature distinguishing the disorders is the tenacity of the delusional belief. Persons with delusional disorder most often will maintain a stable but false belief system for long periods (> 1 month). Alternatively, the threatening beliefs of the person with paranoid personality disorder do not reach delusional proportions and often wax and wane in intensity.

DSM-IV diagnostic criteria for Delusional Disorder



A.
Nonbizarre delusions (ie, involving situations that occur in real life, such as being followed, poisoned, infected, loved at a distance, or deceived by spouse or lover, or having a disease) of at least 1 month's duration.
B.
Criterion A for schizophrenia has never been met.

Note: Tactile and olfactory hallucinations may be present in delusional disorder if they are related to the delusional theme.
C.
Apart from the impact of the delusion(s) or its ramifications, functioning is not markedly impaired and behavior is not obviously odd or bizarre.
D.
If mood episodes have occurred concurrently with delusions, their total duration has been brief relative to the duration of the delusional periods.
E.
The disturbance is not due to the direct physiological effects of a substance (eg, a drug of abuse, a medication) or a general medical condition.
