A 20 y.o. man develops markedly elevated serum lactate concentration and hypoglycemia under fasting conditions.
What of the following enzymes may be defective or abcent in this pt?
a. G6PD
b.glycogen phosphorilase
c. PEP carboxikinase
d. pyruvate kinase

You and your sneaky biochem questions I might be overthinking this question but I think the absence PEPCk might be the answer.

C The absence of pep ck would impair gluconeogenesis, and it might cause pyrucate to build up, preventing the conversion of lactate (coris cycle) back into pyruvate and then to Glucose. This may lead to a large rise in lactate levels. Thats my thinking, though I'm not sure.

I have a Qs .... wich Goljan biochim book ? Rapid Review Series biochim you mean ? If yes, what edition, 1 st or 2 nd ?
Than other Qs : i have the CD of the 1 st edition of Rapid Review biochim, are there any additional Qs in the book itself other then on the CD ?
I will take a look on these Qs no matter what... great sneaky Qs

I think that the answer is a.
The reason why is that why in fasting stages, the liver is going to get into gluconeogenesis in the liver to level the glucose level for energy, the substrate used is lactate.

The von Gierke disease is characterizes by low serum glucose and high serum lactate resulting from deficiency of glucose-6-phosphatase

agree with answer C. Glucose-6-phosphatase deficiency can also cause this situation, but the G6PD in answer a means G6P dehydrogenase. Just need to be careful.

nod PEPCK .. it is .. !! Its not Pyruvate Kinase .. as the Question says that there is an increase in Lactate .... During Gluconeogenesis ... Lactate is needed (Just like Alaninie) and PEPCK form OAA and then the Malate shuttle stuff .. !! So if PEPCK is absent Lactate wil accumulate ... If it were Pyruvate kInase def .. then there would be no Pyruvate formed in teh first place ... and that disease has a seperate set of Symptoms .. mainly Anemia .. !! so hope it clears a bit .. !!
