A 23-year-old female comes in to the emergency department complaining of one day of vomiting and intense abdominal pain. On physical examination, the patient has a diffusely tender abdomen and is tachypnic. You draw a stat basic metabolic panel, which shows Na = 150 mEq/L, Cl–= 108 mEq/L, CO2 = 20 mmHg, pH = 7.2, and bicarbonate = 10. What metabolic state is the patient in?

How do you know the Anion Gap is increased in this case. Please, please explain.

Anion Gap = Positive charge minus sum of negative charge
Anion Gap = Na+ minus [Cl- + HCO3-]
= 150 - [108 + 10]
= 150 - 118
= 32

Normal anion gap
Cl- + HCO3 = 128
Na+ = 140
Difference = 140 - 128 = 12
