A 23-year-old female comes in to the emergency department complaining of one day of vomiting and intense abdominal pain. On physical examination, the patient has a diffusely tender abdomen and is tachypnic. You draw a stat basic metabolic panel, which shows Na = 150 mEq/L, Cl–= 108 mEq/L, CO2 = 20 mmHg, pH = 7.2, and bicarbonate = 10. What metabolic state is the patient in? A. Metabolic acidosis with decreased anion gap B. Metabolic acidosis with increased anion gap C. Metabolic alkalosis D. Respiratory acidosis E. Respiratory alkalosis

Anion Gap = Positive charge minus sum of negative charge Anion Gap = Na+ minus [Cl- + HCO3-] = 150 - [108 + 10] = 150 - 118 = 32

Normal anion gap Cl- + HCO3 = 128 Na+ = 140 Difference = 140 - 128 = 12
