4 hours after meal - already fasting state, metabolism depends on glycogenolysis and gluconeogenesis.
Carnitine deficiency - carnitine shuttle inoperative (transports f.a. from cytosol to mitochondrial matrix).
energy from beta-oxydation of f. a is not available.
Gluconeogenesis is supressed because it relies on f.a.catabolism as an energy source
glycogenolysis is prim source of energy for all tissues
this condition produces hypoglycemia
Treatment often involves fatty acids that contain less than 12 carbons, which can pass directly into the mitochondria without carnitine shuttle
