A 32 year old women gets hyperthyroidism everytime she is pregnant. Serum t4 levels markedly increase during the first 10 weeks of pregnancy. When she is not pregnant, her thyroid hormone status is completely normal. This condition can best be explained by a mutation in the A t3 receptor, rendering it responsive to progesterone. B t3 receptor, rendering it responsive to estriole C TSH, redering it responsive to human chorionic gonadotropin D TRH, rendering it repsonsive to Human chorionic somatomammotropin E TSH, rendering it repsonsive to Human chorionic somatomammotropin Dont forget to explain the reason, answer will come after people try. C. Physiological maximum elevation of bHCG at the end of the first trimester of pregnancy is associated with a mirror-image temporary reduction in TSH. Despite the reduction in TSH, the FT4 levels usually remain normal or only slightly above the reference range. As the pregnancy progresses and the bHCG plateaus at a lower level, TSH levels decrease back to normal levels.

the answer may or might not be correct, is there any relation between BHG and thyroid hormone or TSH?

your right =, the answer is c, this is because lh, fsh , tsh , bhch are all glycoprotein with identical alpha subunits but different B subunits. Mutation in the receptor may allow its activation by bhcg.

Yeah, i will try to pull one question out that teaches some less well known fact per block of questions that I do. You should do that too.. This question was a little easy because you could make the guess depensing on the hormone levels. Besides that I thought the answer was important.

Yeah, agree, we could easily figure it out the answer by guessing a bit , but the Qs explanation, at least tought me how to apply facts ! Nice Qs! I'll try to do same way, though im not through with my whole st 1 material, im doing system wise revision+ Q bank system wise, but i can only learn sooo much from explains! When is your eligibility per ?

I am taking the test in feb. Not sure to the exact date, but have decided to take it before the end of feb no matter what. When are you taking it ?

isn't it Alpha that is same for those hormones, and Beta for specific for each one?
